Office hours during the semester are Monday through Friday 9 am to 5 pm.<\/p>\n<p class=\"p4\">The Wellness Center services are supported by a mandatory health service fee that students pay each semester, thus most services are covered. The health service fee provides each student comprehensive, confidential health and counseling services on campus.<\/p>\n<p class=\"p3\">Please do not confuse the health service fee with the medical insurance fee.<span class=\"Apple-converted-space\">\u00a0 <\/span>These are two distinctly different fees providing different functions. The health service fee covers most services at the Wellness Center, the medical insurance fee covers medical services off campus at hospitals, urgent care facilities, physician offices, etc. The Wellness Center does not bill your medical insurance company.<\/p>\n<p class=\"p4\">The Wellness Center is staffed by licensed and experienced health professionals.<\/p>\n<p class=\"p4\">At the Albany site, students can use the Student Health Center at the Albany Medical Center, 25 Hackett Boulevard, Albany NY (518) 264-0900.<span class=\"Apple-converted-space\">\u00a0 <\/span>Appointments are required and students must bring their medical insurance card for all services rendered and their SUNY Poly ID card.<\/p>\n<h2 class=\"p5\"><b>Health Services:<\/b><\/h2>\n<p class=\"p3\">Sick, injury and preventative care is provided by a physician, nurse practitioner, registered nurses, health promotion coordinator and support personnel. Appointments are recommended, however walk-ins are welcome.<\/p>\n<p class=\"p3\"><span class=\"s1\"><b><i>Medical<span class=\"Apple-converted-space\">\u00a0 <\/span>Services:<\/i><\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li3\">Medical evaluation, treatment &amp; follow-up care<\/li>\n<li class=\"li3\">Women\u2019s and Men\u2019s Health \u2013 GYN examinations, birth control, STD testing, etc.<\/li>\n<li class=\"li3\">Laboratory services: On campus \u2013 strep, mono, HIV, etc., Off-campus \u2013 nationally recognized laboratory pick-up service of specimens for testing<\/li>\n<li class=\"li3\">Medications \u2013 prescription &amp; over the counter<\/li>\n<li class=\"li3\">Medical Equipment\/Supplies\/Other \u2013 crutches, band-aids, cough drops, hot\/cold packs, etc.<\/li>\n<li class=\"li3\">Referrals \u2013 to private practitioners, community agencies, etc.<\/li>\n<\/ul>\n<p class=\"p3\"><span class=\"s1\"><b><i>Health Promotion Services<\/i><\/b><\/span><b><i>: <\/i><\/b><\/p>\n<ul class=\"ul1\">\n<li class=\"li3\">Free, confidential HIV testing &amp; counseling (oral swab with 20 minute results)<\/li>\n<li class=\"li3\">Alcohol\/substance abuse screening &amp;<span class=\"Apple-converted-space\">\u00a0 <\/span>education<\/li>\n<li class=\"li3\">Current health-related educational literature and resources<\/li>\n<li class=\"li3\">Appropriate health guidance with necessary referral (topics include: sleep, stress management, nutrition, etc.)<\/li>\n<li class=\"li3\">Prevention focused programs such as flu vaccination clinics<\/li>\n<li class=\"li3\">Peer education programs<\/li>\n<li class=\"li3\">Smoking cessation assistance &amp; counseling<\/li>\n<li class=\"li3\">Relaxation Room and therapeutic massage services<\/li>\n<\/ul>\n<h2 class=\"p7\"><b>Counseling Services:<\/b><\/h2>\n<p class=\"p8\">Counseling Services play a vital role in the mission of the college by providing a safe and confidential environment for students.<span class=\"Apple-converted-space\">\u00a0 <\/span>Discussion of life\u2019s challenges and the exploration of the complexities of self to enhance the emotional, social and psychological well-being of each student to help them reach their academic goals.<\/p>\n<p class=\"p8\">Counseling is a confidential process designed to help students express their concerns such as college adjustment, anxiety, depression, stress, relationships, sexual identity, self-esteem, etc.<\/p>\n<p class=\"p8\">At the Utica Site, Counseling Services are located in the Campus Center, Suite 217. Appointments are required and can be made by calling (315) 792-7172. All services are strictly confidential and are provided by a licensed mental health provider.<\/p>\n<p class=\"p8\">At the Albany Site, students requiring\u00a0counseling services should visit the Student Services suite, NanoFab south building, Suite 309. All services are strictly confidential and are provided by a licensed mental health provider.<\/p>\n<p class=\"p8\">Questions regarding Counseling Services can be directed to (315) 792-7172.<\/p>\n<h2 class=\"p7\"><b>Other Health Related Information:<\/b><\/h2>\n<p class=\"p3\"><span class=\"s3\"><b>Health Requirements to Attend SUNY Poly: <\/b><\/span><b><br \/>\n<\/b><i>All students at both the Albany and Utica sites, are REQUIRED to provide the following health documents to the Wellness Center before August 1 for fall admission and January 1 for spring. Student athletes and nursing students have more stringent health requirements. Please refer to those respective departments for details.<\/i><\/p>\n<p class=\"p3\"><b>Immunizations \u2013 Mandatory for all students registered for six (6) or more credits.\u00a0Non-compliant students will be de-registered pursuant to the directives of the law.<\/b><\/p>\n<p class=\"p3\"><b>Measles, Mumps and Rubella (MMR)<\/b> \u2013 NYS Health Law \u00a7 2165 requires all students taking on campus classes to provide documentation of immunity to MMR. Persons born prior to 1\/1\/57 are exempt.<\/p>\n<p class=\"p3\"><b>\u00a0Required documentation:<\/b><\/p>\n<ul class=\"ul1\">\n<li class=\"li3\"><b>Measles:<\/b> Two dates of immunization given after 1967 AND on\/or no more\u00a0than 4 days prior to the first birthday<\/li>\n<li class=\"li3\"><b>Mumps<\/b>: One date of immunization given on\/or no more than 4 days prior to the first birthday<\/li>\n<li class=\"li3\"><b>Rubella:<\/b> One date of immunization given on\/or no more than 4 days prior\u00a0to the first birthday<\/li>\n<\/ul>\n<p class=\"p3\"><b>OR<\/b><\/p>\n<ul class=\"ul1\">\n<li class=\"li3\"><b>Titers:<\/b> Date AND positive numeric results of the measles titer, and\/or mumps titer and\/or rubella titer<\/li>\n<\/ul>\n<p class=\"p3\"><b>Meningococcal Meningitis<\/b> \u2013 NYS Health Law \u00a7 2167 requires all on or off campus students provide the following documentation:<\/p>\n<ul class=\"ul1\">\n<li class=\"li3\">One date of the meningococcal immunization given within the past<span class=\"Apple-converted-space\">\u00a0 <\/span>5 years<\/li>\n<\/ul>\n<p class=\"p3\"><b>OR<\/b><\/p>\n<ul class=\"ul1\">\n<li class=\"li3\">Completion of the Meningococcal Information Response Form indicating acknowledgement of meningococcal disease risks and refusal of the\u00a0meningococcal meningitis immunization signed by the student (or student\u2019s\u00a0parent\/guardian if under 18 years old). The Meningococcal Information\u00a0Response Form is available on the SUNY Poly Wellness Center website.<\/li>\n<\/ul>\n<p class=\"p3\"><b>Health History and Physical Examination within the last two (2) years \u2013 Mandatory for all students registered for twelve (12) or more credits. <\/b>A SUNY Poly student<b> <\/b>may only receive clinical services at the Wellness Center once<span class=\"Apple-converted-space\">\u00a0 <\/span>the health history and physical examination are on file. Full-time students\u00a0will not be permitted to register for another term until this health requirement has been met. Students taking less than 12 credits may use the clinical services if a physical examination within 2 years is on file<\/p>\n<h2 class=\"p2\"><span class=\"s1\"><b>Medical Insurance<\/b><\/span><b> <\/b><\/h2>\n<p class=\"p3\"><span class=\"s1\"><b>Domestic Students<\/b><\/span><b> &#8211; Medical insurance is mandatory for all students at both the Albany and Utica sites who are registered for twelve (12) or more credits and all nursing students,\u00a0regardless of credit hours.<\/b>\u00a0SUNY Poly provides a basic, medical insurance plan for students who need coverage or wish to purchase additional coverage. Part-time students registered for at least 6 credits per semester may request enrollment EACH semester by the first day of class.<\/p>\n<p class=\"p9\"><span class=\"s1\"><b>FALL ADMISSION: <\/b><\/span><\/p>\n<p class=\"p10\"><b>All full-time domestic students\u00a0(12 credit hours or more) and\u00a0all nursing students regardless of credit hours,\u00a0are required to purchase the SUNY Poly designated medical insurance IF they do not have other medical insurance coverage. \u00a0<\/b>If you have other insurance and wish to waive the SUNY Poly designated medical insurance, you must complete the on-line waiver\u00a0by the first day of class\u00a0and\u00a0EACH <b>FALL<\/b> SEMESTER\u00a0thereafter, otherwise the student will be automatically enrolled AND billed for the SUNY Poly\u00a0designated\u00a0medical insurance.<\/p>\n<p class=\"p10\">Failure to submit the waiver will result in the student being enrolled and billed for the SUNY Poly designated medical insurance plan. <b>The premium is non-refundable and no waivers will be accepted after the first day of classes.<\/b><\/p>\n<p class=\"p10\"><b>Submission of the waiver in the fall semester waives the medical insurance for the entire academic year, fall and spring semesters.<\/b><\/p>\n<p class=\"p10\">Your username and PIN number are required to access the on-line waiver. All newly registered students receive their username and PIN number through the mail prior to your first semester.<\/p>\n<p class=\"p10\">Within 3 business days of your\u00a0medical insurance waiver submission, an e-mail is sent to your SUNY Poly e-mail address confirming that you have not been billed nor enrolled\u00a0for the medical insurance.\u00a0 Your tuition\u00a0bill\u00a0will\u00a0indicate no charge for the medical insurance once you have received the e-mail confirmation.<\/p>\n<p class=\"p10\">Always check your tuition bill.<span class=\"Apple-converted-space\">\u00a0 <\/span>Direct your questions to the Wellness Center, (315) 792-7172.<\/p>\n<p class=\"p11\"><b>Online insurance waiver site:\u00a0<a href=\"http:\/\/www.sunypoly.edu\/healthwaiver\" target=\"_blank\" rel=\"noopener\" data-saferedirecturl=\"https:\/\/www.google.com\/url?hl=en&amp;q=http:\/\/www.sunypoly.edu\/healthwaiver&amp;source=gmail&amp;ust=1529679660762000&amp;usg=AFQjCNEw8gIpXOxyHn2NKHnuksi9BxoyFQ\">www.sunypoly.edu\/healthwaiver<\/a><\/b><\/p>\n<p class=\"p12\"><b>Please have your medical insurance information for waiver submission: insurance company name, address, subscriber ID number, etc.<\/b><\/p>\n<p class=\"p10\">If your insurance needs have changed in the fall semester and you wish to purchase the medical insurance for the spring semester, please contact the Wellness Center (315) 792.7172 by the first day of class of the spring semester.<\/p>\n<p class=\"p10\"><b>Part time students<\/b>\u00a0(6 to 11 credits) who do not have\u00a0medical insurance can purchase the SUNY Poly designated\u00a0medical insurance plan by requesting it through the Wellness Center\u00a0(315) 792-7172.\u00a0Part-time MUST REQUEST ENROLLMENT <b>EACH <\/b>SEMESTER.<\/p>\n<p class=\"p10\"><span class=\"s1\"><b>SPRING ADMISSION:<\/b><\/span><\/p>\n<p class=\"p10\"><b>All newly enrolled full-time students (12 credit hours or more) and\u00a0all nursing students regardless of credit hours ARE enrolled and billed UNLESS they complete the on-line waiver which remains in effect for ONLY the spring semester.<\/b><\/p>\n<p class=\"p10\">If you choose to waive this coverage, you must do so before the first day of\u00a0classes\u00a0for\u00a0the spring semester.
