Forms and Helpful Benefit-Related Telephone Numbers
Many frequently used forms such as Health Insurance, Dental, and Vision Care Claim forms; Retirement, Flex Spending, and Tuition Waiver forms, are available on the Human Resources website at: http://www.sunyit.edu/human_resources/forms.inc .

SUNY Perks
SUNY Perks is the employee discount and savings program that provides offers from a wide variety of merchants and vendors. SUNY Perks gives you access to private shopping events and exclusive discounts on hundreds of brand name products and services. Recent program enhancements include:

• Click on the Register Now Button
• Click “If you do not have a SUNY email, click here”
• Enter your alternative email address and click Submit
• Enter “sunycp” as your company code and re-enter your personal email address. Your password will be sent to you in a couple of minutes.

NYS-Balance

The NYS-Balance Program is a confidential resource and referral service designed to save employees time and help them better balance work and life. It is available 24 hours a day, seven days a week. Employees and their families can call a toll-free number or email a consultant for assistance with a wide spectrum of needs including:

• Finding a home repair person
• Living within a budget
• Finding child care or a summer camp
• Living healthier
• Coping with elder care issues
• Planning for retirement
• Preparing for college
• Parenting strategies

In addition, the NYS-Balance website provides an abundance of useful resources, articles, webinars, online courses and interactive tools.

To log in to the NYS-Balance site go to: www.nysbalance.ny.gov, enter username: nys and password: balance.

BJ’s Membership
Applications are available in the Office of Human Resources for NEW and RENEWING BJ’s Wholesale Club Memberships.
This Inner Circle Corporate Membership is available to our employees and includes a $10 savings off the $50 annual membership, a free second card for a household member and three free months. The cost of the 15-month membership, for two-household cards, is $43.50. BJ’s sends out renewal notices approximately 1 month before expiration date.

To apply for membership (both new and renewing), you must complete the application and mail the application and payment to our BJ’s representative (HR will provide pre-addressed envelope). Please DO NOT bring application/payment to club location, it must be mailed.
New Members- It may take up to four weeks to receive your membership letter in the mail. Once you have this number, you will need to stop by any BJ’s Wholesale Club for your card. A temporary membership card is on the back page of the application but this temporary membership card does not work at the gas pump.
Renewing Members- You will not receive a new membership card. Your membership will be extended with the new expiration date. If your membership expires before the extension is applied, just use the temporary pass on the back of the application until your card has been updated (please be aware that this temporary membership card will not work at the gas pump).

SUNYIT Online Directory

The SUNYIT website has a link to an online directory for faculty, staff and students. This directory lists such information as office/room locations, telephone numbers, and email addresses, etc. Staff have the ability to update their information in this online campus directory any time a change is necessary. Here are some basic directions to do this:

(1) Go to www.sunyit.edu
(2) At the top of the page, click on “Directory/Search Tools”
(3) On the left side, click on “Tools, Update your Bio”
(4) You will need to log in (same login that is used to access your SUNYIT email)
