To change your health insurance option for 2014, you must complete a NYS Health Insurance Transaction Form.  These forms are available in the Human Resources Office.  If you have any questions regarding the health insurance option transfer, please contact Audra Cornelius, Benefits Administrator, at ext. 7194.

2014 Health Insurance Opt-Out Program

In 2014, NYSHIP will continue to offer the Opt-Out Program, which allows eligible employees who have other employer-sponsored group health insurance to opt out of their NYSHIP coverage in exchange for an incentive payment.  The incentive payment is $1,000 for opting out of individual coverage or $3,000 for opting out of family coverage on an annual basis.  The incentive payments are prorated and added to your bi-weekly paycheck as taxable income.

If you are interested in participating in the Opt-Out Program, you must complete a NYS Health Insurance Transaction Form and an Opt-Out Attestation Form.  These forms may be obtained in the Human Resources Office.

If you currently participate in the Opt-Out Program for 2013 and wish to continue to receive incentive payments, you must elect to opt out for 2014.

The deadline for enrolling or re-enrolling in the Opt-Out Program is December 31, 2013.
