Registrar: Readmission Application – SUNY Polytechnic Institute Forms

Application Deadlines are as follows:

Readmitting for the FALL semester: August 01 (Candidates seeking readmission to the Family Nurse Practitioner program are encouraged to apply by February 01).
Readmission applicants seeking second degree admission to the PMHNP DNP + CAS programs must submit their application by April 1st. The program is a fall start only.
Readmitting for the SPRING semester: December 15
Readmitting for the SUMMER term: May 01

This application form is the form you must submit in order to be considered for readmission/rematriculation to SUNY Polytechnic Institute. You have the option to be readmitted:

Into the same program that you were previously enrolled in or
Into a new degree or
Into a different program (changing your major), or
To complete a second major.

To be readmitted, you must meet the admissions and program requirements in place for the semester in which you intend to re-enroll.

The readmission application is term-specific. It cannot be transferred to a future semester. If you apply for readmission and do not attend, you will have to resubmit this form for a future semester and submit another readmission application fee.

Submit this online form and pay the application fee. All applicants must pay the (nonrefundable) readmission application fee.
