Complete this form to change a major, add/change/delete a second major, add/change/delete a minor, or add/change/delete a concentration. The addition of a major, minor, or concentration must be signed off on by the Program Coordinator.

  • Complete the appropriate section(s) below based on your current academic level and request(s). Submission of this form implies that you approve of the requested change(s). Once submitted, the form will be routed to the Program Coordinator for approval. The Registrar’s Office will notify you if the Change of Program has been approved or denied.

  • UNDERGRADUATE STUDENT

  • Enter the name of the second major you are adding or removing. If changing from one to another, be sure to indicate the program you are changing to and from.
  • Enter the name of the minor you are adding or removing. If changing from one to another, be sure to indicate the program you are changing to and from.
  • GRADUATE STUDENT

  • Enter the name of the 2nd degree you are adding or removing
  • Enter the name of the concentration you are adding or removing. If changing from one to another, be sure to indicate the program you are changing to and from.
