    This form is to be used to request waivers of college policy or for course substitutions. To request to take a course at another college, please use this request form https://forms.gle/kLZvLq87E9J1j9nj7 Instructions: Be specific and include all relevant information to support your request. Attach additional sheets as needed. The completed petition will be sent to your Academic Advisor/Department chosen, who will forward the form to the Registrar's Office. The Registrar's Office will obtain additional approvals if required.
