F-1 Student: SEVIS Transfer Request

This form is for any student who entered the U.S. with a SUNY Poly Form I-20.

Any student who entered the U.S. with a SUNY Poly Form I-20 and wishes to transfer their SEVIS record to another school must submit this form before their transfer will be processed.

**DO NOT SUBMIT A TRANSFER OUT REQUEST UNLESS YOU ARE 100% CERTAIN OF YOUR TRANSFER SCHOOL & RELEASE DATE.

Once you submit this request, your transfer will be scheduled in SEVIS for the date you indicate.

Do not submit this request to the ISS until you are certain:

That you wish to transfer and leave SUNY Poly
The school you wish to transfer your record to
The date on which you want this transfer completed.

Please allow 5-7 business days for processing of your request.

*There is no need to contact the ISS to inquire about the status of your request or to inform us of your submission. Once your request is processed, you will be notified through your SUNY Poly email.*

STUDENTS MUST WITHDRAW FROM COURSES WITH THE OFFICE OF THE REGISTRAR.
