F-1 Student: Travel Signature Request

This form is for students currently enrolled or engaged in OPT/STEM OPT at SUNY Poly only.

F-1 Students are required to have a valid travel authorization from their school P/DSO on their Form I-20 every time they enter the U.S.

Travel Authorizations are provided on page 2 of the Form I-20, and are valid for 12 months for students enrolled in classes, and 6 months for students engaged in post completion OPT/STEM authorization.

Students should not submit this request if they have a current, valid, travel signature on their current Form I-20.

Please allow 5-7 business days for review of your request.

*There is no need to contact the ISS to inquire about the status of your request. Once your request is processed, you will be notified through your SUNY Poly email.*
