F-1 Student: OPT STEM Extension Request – SUNY Polytechnic Institute Forms

This form is for SUNY Poly graduates with APPROVED post completion OPT authorization from USCIS.

Students who are currently engaged in Post Completion OPT authorization may be eligible for the 24 month STEM OPT Extension authorization.

This request may be submitted up to 90 days prior to the end of the current Post Completion OPT authorization, and MUST be submitted AT LEAST 7 business days prior to the last day of the current Post Completion OPT Authorization.

The STEM OPT Extension CANNOT be requested after the current OPT authorization has ended. It is the responsibility of the student to ensure their request and application is submitted in a timely manner.

The ISS may take 5-7 business days to process your request. Once this is processed, you will receive your requested Form I-20, as well as further instructions to complete your application, at your SUNY Poly email address.
