F-1 Student: OPT Requested Form I-20 Application – SUNY Polytechnic Institute Forms

This form is for students currently enrolled at SUNY Poly only.

All F-1 Students seeking Optional Practical Training (OPT) authorization (pre or post completion), must submit this form for issuance of a Form I-20 with OPT Requested prior to submitting their application for OPT to USCIS.

Applications for OPT are not complete until the student has fully submitted all requirements to USCIS as instructed.

The ISS may take 5-7 business days to process your request. Once this is processed, you will receive your requested Form I-20, as well as further instructions to complete your application, at your SUNY Poly email address.

*There is no need to contact the ISS to inquire about the status of your request or to inform us of your submission. Once your request is processed, you will be notified through your SUNY Poly email.*

Do not submit this form unless you are 100% certain of your graduation date and your desired OPT Start Date.

Your requested OPT start date cannot be changed once your application has been submitted.
