This form is for students currently enrolled at SUNY Poly only.

All F-1 students wishing to apply for Curricular Practical Training authorization must submit this form.

Please allow 5-7 business days for review of your request.

*There is no need to contact the ISS to inquire about the status of your request. Once your request is processed, you will be notified through your SUNY Poly email.*

You may NOT begin employment until you have received your Form I-20 with CPT authorization on page 2 from the ISS, AND the start date listed on your Form I-20 has arrived.
